How to register

Please note: It can take up to 14 days for you to be fully registered with the practice. Please bear this in mind should you try to submit an eConsult before your registration has been fully put on our system

Text description of boundary area – From Haymarket Station along West Maitland Street, Shandwick Place, Princes Street, North Bridge, South Bridge, Nicolson Street, Clerk Street, South Clerk Street, Newington Road, Salisbury Place, Grange Road, Beaufort Road, Strathearn Road and Strathearn Place, then heading directly to the west end of Chamberlain Road passing through the bend on Greenhill Park, along Colinton Road, Gillsland Road and Harrison Road, continuing due north-west to the south end of McLeod Street, then north and east along the railway line to Haymarket Station .

ONLINE REGISTRATION FORM

You must bring photographic ID with you, the first time you attend the practice. 

If you have one, please bring along your NHS medical card and two pieces of formal identification. One piece of identification should include your photo and the other should give details of your new address.

Photographic ID could include – passport, driving licence or matriculation card.

Documents with proof of address could include – utility bill, medical card, bank statement or letter from solicitor or landlord confirming address.

You have the choice to see a particular Doctor, if available – please inform the receptionist if you have a preference.

We do not exclude patients from the Practice list on grounds of race, sexuality, religion, social class, age, disability or medical condition.
