If you have a medical concern which you wish to discuss with a doctor, please click on the ‘Contact your Doctors Online’ form below. This service is open Monday – Friday 08:00 until 16:00.

If you have issues using our ‘Contact your Doctors Online’ service, please contact reception on 0131-536-9700 where a member of the team can take you through our eConsult lite service.

If you have a medical concern outside office hours, please contact NHS24 on 111.

If you would like to register with the practice, please click on the patient registration tab above and complete the online registration form. If there are any issues with the online registration, please contact reception.

We are experiencing a very high level of demand at present. This may continue for several weeks and it may take longer than usual for you to get a reply. Any routine request, such as sick notes or routine prescriptions, may take longer than usual to process. If you feel that your symptoms are worsening and our response is taking longer than expected then please phone reception to highlight this. Please consider phoning NHS24 on 111 after 6pm if you feel you need to have an assessment this evening. Leven Medical Practice.

Allow 48 – 72 hours between request and collection from your preferred pharmacy.
